We need help in creating a short test /questionnaire - only 9 items. Test description: Questionnaire with the selection criteria for inserting data. The concept is to have a standard report with prewritten insertions to be included depending on the answers. For example, the report will have a standard sentence about people (question 1) which will be replaced by insert A, B, or C depending on the answers as stated below. Other notes: Example - Each heading is a screen, and each heading set of answers produces three possible responses. Php10000-16000 for project work. Timeline: 8-12 days Work must before quality approved before payment will be made. Must share sample of similar work to be awarded task.
