Need A design of a Raspberry Pi Pi Hat - The HAT will be a UHF Reader module + POE

Hello, I would like to integrate the reader module onto a PCB board that can connect to the Raspberry Pi. The HAT will also need to have a serial to TTL connection that connects to the readers GPIO1 and GPIO2 pins. The HAT will also need a POE device that can power both the reader and the Pi. I'd like to use the si34061 POE chip to accomplish this. I basically need a Need a board-level schematic to connect POE and UHF Reader module to the Raspi Pi 3+.
