I want the forms on my website re doing and the database needs changing as the hosts are moving everyone to a new database. If it is possible I want the answers to the forms to be converted to a word document with logo on it etc so I can simply send it on.

this isn't a difficult job and I could do with it being done urgently. I will supply all info and the logo's etc
