I am a student and I have to submit an assignment at school with a good looking, feel good design of a simple web page. This simple design interface asks the user some 5 survey questions. with Yes NO Decline Option check boxes. This page should have a header where the description of the survey is given. For these questions user can see his progress of answers through a color ful animated progress bar with percentage% of progress. and under that I need a group of buttons or navigating small numbers 1 to 5 which should navigate to appropriate question. The questions panel should be below the progress bar with next and previous option which should take to next and previous question.
