Landing Page(s) for PPC Campaigns

Landing page development for pay per click campaigns. Need page development which will include an online questionnaire/application. Additionally, I would like to integrate a mortgage calculator into the page. I already have a website and calculator code. The final product will allow me, someone with basic html skills, to make changes to the page's content as our campaigns are very dynamic. I will want to include the calculator on the landing page sometimes and other times we may want to eliminate it all together.
