JQuery & AJAX for Single Page App

I have a form that has 5 inputs. I need someone to implement a JQuery function to grab the values, validate them, parse them (some inputs), do some simple calculations, and then update 2 separate divs with the calculations and another div with an AJAX call to a database table for 1 field. This is probably the easiest thing in the world for an experience Javascript/JQuery developer but I'm absolutely swamped with clients and other things throughout the day to dig into this. I have all the calculations mapped out to their corresponding inputs so the pseudo-code is already taken care of - I just need it interpreted into working code. No special transitions or animations needed for this project. I'll supply a diagram with some plain english for the person who accepts.
