I'm selling a website. Need a simple contract written.

I'm selling a website for a fixed sum of $x and x% of the profit for the next x months. I need to make sure I get my share of the profits sent to me no later than 2 weeks after the end of each month for x months. I need to transfer the domain, website, data and brand to the new owner. I also need to limit my liability and basically say that I'm not responsible for anything after the transfer has been made. Also any other bits you think I should add.
