Simplified Craigslist-like Report Listing Plugin for SOBI2

I would like a very simple Craigslist-like report listing feature in the Sobi2 vCard template. I want a drop-down menu in the vCard template that allows the user to pick a reason for reporting, then a "Send" button to submit a report to the administrator email address. [login to view URL] do make a free Sobi2 plugin called "Report Listing" but it does not work as described above. The existing Sobi2 Report Listing plugin takes the user to a separate page where the user is required to fill out his/her name, contact info, message, and captcha. I do not want this. I want a more simple functionality than this. Perhaps the existing Report Listing plugin can be modified to meet my requirements. Here is a mockup design I created.
