Altering the look of an online form

Have an established online form that needs a few adjustments: Insert a new graphic (already designed) for a new header and put into login page also Make form fit an a standard printed page size - at the moment it is a little short when it has little writing in it. When the form is saved as a PDF there is a tick box that gets dropped - this needs to stay when the PDF is made
