186017 simple PHP data submit form

Hi! I need a very basic PHP script, that shows a form like this: ---------------------------------------------------- Enter a list of domains: Niche (keyword): ________________ Domains list: _______________ (each domain in new line, separated by line breaks) - Submit button - ---------------------------------------------------- If one of the fields is empty, the script should display a message in red - "Please input data. All fields are required!" (and display the form again, right below the error message). If data is entered correctly, the list of domains should be written (as CSV file) to sub-folder called /List/ , which is located in current script folder. The new CSV file should be named after the submitted content of "Niche" field - e.g. Niche_mmddyytime.csv. When the script has completed writing the file, the screen should display a message "Data Saved Successfully!" A link to download the file should be sent to admin email. -- The main form must be password protected - I shall be able to access it after entering login and password, editable in script config file.
