PSD to Live Site with Form

We are creating a website for our Translation Business. It is a simple website with an interactive form (Form is most important aspect). We need: Website built in HTML/CSS. The form built. Ive attached a couple JPGs of what the website will look like, and a link to a Vimeo Video of how the form should work: Video Link: [login to view URL] Password: plaintranslations Note about the form: - The form should work like shown in the video. The form will finally lead the customer to a paypal payment page, and the customer will pay through pay pal. - After the customer pays through pay pal they would be taken back to our site where they will be greeted with a thank you page. We need all the information the customer enters in the form and the documents they upload onto the form to be emailed to us so we know how to translate stuff for the customer. - Once the customer has payed successfully, and the data has been emailed to us successfully the customer should get a confirmation email. Question: I am not sure when the data from the form should be sent, upon payment or when the customer clicks the Pay now button. It would be more useful if the data was sent only once the customer pays because we would only work with the data once the customer was payed, what are your thoughts and is this possible.
