CC.PH2.02.01: Web update 10/25/2013

SCOPE: I’m looking to update an existing form. The website is development on PHP/AJAX/JavaScript/MySQL. You will have access to a test site for development. The project is to add nine (9) fields to an existing form. • Field 1 (varchar) • Field 2 (varchar) • Field 3 (varchar) • Field 4 (varchar) • Field 5 (varchar) • Field 6 (varchar) • Field 7 (varchar) • Field 8 (varchar) • Field 9 (longblog) All fields have already been added to the database. All the work will be on the backend. Work must follow existing template style. There is no work on the front-end. Developer should have knowledge of PHP/AJAX/ JavaScript /MySQL/CSS. During the selection process, I will provide full requirements to qualified developers. This will allow you to fully understand the scope and determine if there are any issues. DESIGN CONCEPT: Following existing design template. HOSTING: Hosting is on [login to view URL] PAYMENT STRUCTURE: Money will be Milestone and release upon the following: 30% - upon completion and approval of development 70% - upon successful final UAT and final delivery of all source codes and files This payment structure is a pay as you go method and will reduce risk for the developer and ensure progress for myself. COMMUNICATION: I require someone who will actively communicate with me and update me throughout the project. TERMINATION: This project will be terminated if I feel the development and/or communication process has stalled. If there is an issue, identify it and let me know. I need to turn in a status report as well. OWNERSHIP: Upon full payment, full worldwide ownership of the software/application will transfer to me.
