Create Joomla 2.5 Form for booking dates & paypal payment

I need a very good form create for a Joomla website, Using whatever forms component you prefer. The form will work as a booking system for a holiday camp website, with PayPal payment at the end. You will need to create only 1 location camp booking form, but I need the ability to copy this form for different locations. Back End: 1. We need to be able to manipulate the dates for the camps that we run so that we can re-use the form in future. 2. We need to be able to pull the data into a tabular format. 3. It would be nice (not essential) if there was a way of filtering the table (so that we can produce print outs of daily attendance). 4. The prices of our camps vary based on location, so we need to be able to set different prices per copy of the form that we use. 5. We need to be able to set discount (e.g. 5% discount for siblings that sign up and a set price for a booking of 5 consecutive days ) Front End: 1. Form should include the following criteria... a) The dates that the customer wishes to book b) The customer first name, surname, email address and mobile number c) The child's first name, surname, DOB and any medical information d) The ability to add multiple siblings to the same dates or different dates e) The user will be sent to Paypal at the end with the total cost for the amount of days & amount of kids
