Write some code to take orders on a page and save them

Hi, We are taking orders for products on the phone and need a system to keep the orders coming and writing them. We need to write a small page where we can input Items like the following - Name: Place: Phone Number: etc. Then we proceed to taking the order where we can have grids displaying the products which can be selected. After selection, we can also show the total value of goods based on numerous rules. Also, we would like to build in a way to calculate the total weight of the ordered goods and thus compute the freight. Its a small one page code, would be good to have the output in the form of csv/ excel continuously. Better still, be able to work parallel with google forms.
