htaccess Login script with html form authentication

I need a login script with basic authentication using htaccess. A standard htaccess login script displays a standard login popup that is unstyled. I need a login script that uses a html form that can be changed. When i protect a directory with such a script the following should happen: when i enter "http://protected_dir_url/[login to view URL]" when not logged in i get a login form. when i enter "http://protected_dir_url/[login to view URL]" when logged in i get to see the image. in this case [login to view URL] can be anything even a subdirectory so it should behave like a regular htaccess protection script. and i need a script to logout, if this script isn't used the login is remembered until the browser closes. the user and password are set in the php file, it should be a simple script without database.
