Make small changes to a plugin VERY QUICK WORK

The attached plugin was designed to send emails from a wordpress dashboard. I need it changed to only send emails to the admin email address. So there should be no "To:" field. Also there should be no subject field and instead of a text area field for "message" it should say "Select single backgound image:" With a drop down menu with two options "Master" & "Variation". There should also be one last option "Submit your own background image (must be at least 1200 X 800 and 1MB max)" with a submit file option. That is all. Anyone who knows what they are doing can get this done in 15 minutes. I will not pay more than $10. Thanks!
