Need a bootstrap page php wizard that does the following 1. First page will contain two prompts ( DRIVER / VENDOR ) 2. Second page will ask the DRIVER OR VENDOR for telephone number in XXX-XXX-XXXX 3. The third page will query a MYSQL database for the drivers or vendors TELEPHONE number and if they have done this before the PAGE will display WELCOME BACK (NAME) or if they are not in database it will ask them for the following information If driver - Name and carrier (select from drop down) If Vendor - Name and company (fill in text field) The 4th Page for the driver will have the name / carrier name and ask for a trailer number and load number. The 4th page for a vendor will have the name / company name and ask for the reason of visit. At the bottom of the 4th page will be a button that says CHECK IN. This data will be saved in a MYSQL table :) and the design should be response and BIG for a kiosk monitor.
