Write some Software Based on a sample that you can use as well

Hello, I need simple script put together. I have everything that you need to put stuff together (most of it). This is script source (you can use all of those files to put stuff together, I don't mind): [login to view URL] This is demo: [login to view URL] Now, what I need done is following. #1 Instead of home page I need to put it together nicely like (take ANY template from the web) to have nice SEARCH button (and search function). #2 Home page should have some nice search glass on it, adding records button and simple browsing button. #3 I need ability to ADD/EDIT/REMOVE records. #4 Search is very important part of it and abiliti to list by FIRST letter of last name. #5 Pagination should be there as well just as on sample just to look nice and fresh :) #6 Database should be UTF-8 having ability for čćžš etc.. as well as searching for it. Pages are: Home page Add records Edit records Delete records (while browsing) with javascript confirmation if you can't add that, I will. Search should be done from the very home Browse (by letter) Regarding database, this is structure as well as mandatory fields marked with * when it comes to adding data: ID FirstName* Middle name LastName* ChartNumber* (up to 1-6 digits max) Phone Comments Make that using/editing script above or do it on your own I don't care :) Put it together in some ligth css or whatever design.
