CCBill form and quick tutorial

I am having trouble creating a simple form in CCBill. I require a simple basic form created in my CCBill sub account with price option added. Having read the CCBill tutorials I still find it ridiculously confusing so the work would also be to include a brief tutorial on how it was done so I can add more in the future. Thanks
