VB Script for Active Directory needed

Can anyone develop a small script for us to add a new user to Active Directory. Script should use the following input boxes: Firstname: Lastname: Password: Not mandatory, but prefered: Tickboxes to specify Usergroups the user should belong to as well as Organizational Units. When entering those details, the script creates automatically a user in the Active Directory (password needs to be changed upon login) with the username based on firstname.lastname. Another feature which would be good is creating a folder on another server (homefolder) based on username. User, who using this script will have permission to run the script. NO webserver is availble, no IIS or other application. Only Windows Scripting should be used.
