Professional Website Template with Logo & Header Graphics

We need a very professional looking business website template. We have a number of sites that we want to use to build our brand with this template. The template needs to include the following: - A professional looking header with our business name and a logo (you will need to design the logo). - Other basic website template stuff like sidebar, footer etc. We will also need additional widgets/styles that we can add to the pages as needed. These will include boxes to display summaries of our products, images for buttons, styles for forms, lists, displaying images, header tags etc. We'd also like a slick way to display images and samples of our products, perhaps a thumbnail that enlarges as you hover the mouse over. We will need all the PSD source files so that we can alter the text in images like the header, buttons, price etc. The template should be written using html/css and javascript if necessary. We are looking for someone who takes pride in their work and creates polished looking website templates. Could you please include some links to your previous work on websites and templates.
