designing work of a script

Hi, I need a talented designer to design th estatic interface of a script I'm editing , it's an uploader and the interface is like a control pannel like thw windows control pannel or the Cpanel so it's just small icons and names + nice logo to th escript + design of a form apperance(plus the control pannel the tool includes some forms and it geneates upload forms so there will be like 4 forms needs good looking) every thing is well-documented to easily understand and implement what I want
