Design and formatting of 'whitepaper'

I have a company whitepaper of approximately 3,500 words, setting out our company's position on how to solve a particular business issue. My company is a professional consultancy in a HR related field. I am looking for this whitepaper to be given a professional and attractive cover, text formatted, and a booking form created. Logos, colour schemes, fields and text for form etc. provided. ## Deliverables a.
