Excel Spreadsheet Data Entry

For this data entry project, I have two excel spreadsheets that need to be populated with data from a PDF. The first spread sheet is roughly 500 clients with roughly 5-6 fields of data entry. The fields are firm name, street, city, zip code, phone number, fax, website.
The second is roughly 4,000 contacts with the following data entry fields: first name, last name, email, firm name, street, city, state, zip, phone, fax.
I have posted two examples of the data format you will receive. They will come by PDF. I will supply the spread sheets and the fields I require. If you have any questions please ask. I am looking for speed, accuracy and quality.
