small php/ajax mysql database script(repost)

I have a mysql database (the SQL is attached) I need a php file that will display a table on a webpage. I also need an ajax input page for an operator to enter the data into the database. After the data has been entered the ajax page should show the table, with the option to: view by short name, add a new record, amend a record or delete a record.
