Facebook Multi-choices with result Application

So it's quite straightforward, this should be a Facebook Application with the following characteristics: * People have to click "Like" to start using it. * The application ask to share on people behalf (the end result). * Then you have 4 questions with 2 answers each (the following question is displayed after question is answered and people clicked "next") * The application should support UTF-8. * There is a set of 16 answers to be displayed as results (1 for each questions choices combination) * The application asks if people want to share the application itself to friends at the end. We can see plenty of those application all-over Facebook. I need the application to be created and set up into a Facebook page. Don't send me a copy/pasted message until you made plenty of those applications and this is clearly written in your feed-back.
