488753 Fillable Word or PDF Form

I need either a PDF or Word document that is fillable. It is only 1-page and will have about 20 fields of data. I need someone that can design the form nicely, and create fields that can be filled in by users. The form will not be online. It will be emailed to users. The form will be a purchase order/quote form. PLEASE NOTE: Design layout is important here. You will need to create a table and or layout boxes nicely and choose a nice color scheme. We looked online for a template, but nothing matches perfectly, so please do not bid unless you can create the form. Attached are a few sample style documents, but what we want is different. The sample are juat intended to show you that the design, color scheme, layout, and functionality are very important. Must be able to complete within 24 hours of award. Thanks!
