Adobe Reader Form Modification

In our company we use a form to fill the daily work. The form must be lightly modified. It's about 2 hours of work. I attache file [login to view URL] where is original document (still not finished) and new file [login to view URL] where the change should be applied. The field Name is a drop down list. The list values must be preserved. The Datum is just a date entry... The fields Project should have names Projekt01... Projekt 04. The List values must be preserved. The Fields von should have name von01.0 ... von01.6, von02.0 ... von02.6, etc. The Fields bis should have name bis01.0 ... bis01.6, bis02.0 ... bis02.6, etc. The calculated field Stunden should have name stunden01.0 ... stunden01.6, stunden02.0 ... stunden02.6, etc. The needed javascript calculation is already incorporated. The Fields text should have name text01.0 ... text01.6, text02.0 ... text02.6, etc. The field stundenZusammen ist just a sum of all the stunden fields. All the fields must be 100% aligned and with uniform sizes. Thats all what should be done. Offered price is 30 euro.
