We require a contact form that includes the following: - Fields for Name, Address, Phone Number, Email Address - Field to include an attachment with a size limit - The form must be able to be sent as an email to a specified email address *with* attachments - Preferably some nice styling, such as with CSS and Scriptaculous or another javascript framework - Ideally, some kind of managment console for forms submitted would be a plus, but not required What must be delivered: - All files, including any processing scripts
