PHP and MySQL db application edit

UPDATED: I HAVE CHANGED THE PROGRAMING LANGUAGE FROM PERL TO PHP DO TO CHANGE IN APPLICATION PURCHASE. I am asking to have a recently purchased PHP "membership management" program called PLEXUM [login to view URL] - modified. I have permission from PLEXUM to do so. The program automatically recreates a copy of a web page when a new member joins our organization. I need some links and a small amount of information from the MySQL database to be displayed on the new replicated web page. I have attached a PowerPoint that spells out just what I need to have done above. I also need a simple form created when a visitor comes to one of the pages (the directory) and click on a member's page, before going to that page I want to collect some basic info from the visitor first (ie. name, email, tel#) and have that info entered in the MySQL db and emailed to the member of owns the page the customer is visiting. The PLEXUM software has some basic email ability but I need to automate the sending of this info. I also need good documentation of what code you may have written and any changes you made to the existing code of the application should be have to "back it out" do to future application upgrade issues. FUTURE OPPORTUNITY: There will be significant additional contract opportunity for the right programmer as this project will require continuous ongoing modification. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 1.1) Detailed documentation on any code written or any changes to the existing application. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement). ## Platform I believe it will be running on the Unix/Linus server. Web Server Requirement: 1. PHP 5 or above compiled with CURL, and XML support. 2. Zend Optimizer 2.5.5 or above. 3. CURL, (mysqli or mysql ), FTP , XML and GD2 extensions support. 4. SSL enabled Web Server for Recurring or Auto Billing capability 5. MySQL 4.0.0 or above. 6. Ability to run [login to view URL] as the index page default. 7. Full execute, read and write permissions under document root directory for your domain account. 8. 50 mg minimum of web space. 9. Mail Program: Sendmail or SMTP . 10. CronTab (not so required as Plexum X5 have a built-in FakeCron but CronTab is preferable). 11. DNS-Wildcard (Only required if you intend to use Sub-domain add-on module). MySQL Database: 1. 20 mg minimum space. 2. Must have database Hostname, Database Name, Username and Password. 3. The database user must have full privileges to: (Lock/Unlock Table, Read/Write, Table Alter, Drop Table). 4. DB Must only be used for Plexum X5
