provide html5 video embed code

We have a video available in 4 formats: mp4, webm, ogg, and f4v. Size is 640 x 360 I need you to provide me with the embed code to put on our website so that the video is viewable by the maximum number of people and browers, ie including: IE 9 & 10 Chrome Firefox Opera Safari Ipads Androids Iphones, windows phones, etc with and without javascript enabled The video should also automatically resize to fit the device. In addition, I want the following parameters: autoplay: true At the end of play, the video should either just display the final frame, or else a splash image I want to embed google analytics so we can see how much of the video is being played. The controls should appear only on mouseover, as we currently have them on our site. I am happy to use flowplayer free version. Our site is coded in php with a header file and a page file. This is a really simple project for anyone who knows what they are doing. I am looking for someone who can do it right away, ie now. I expect it can be done in less than half an hour
