Wordpress Form

We want to add a form to our Wordpress site which allow restaurant owners to add their restaurant for free listing or that we can charge them a montly fee for their listing. For example, please see h t t p://templatic.c o m/demos/geoplaces3/?ptype=post_listing HOW IT WORK The restaurant owner sign up at the website and add their listing, and select payment plan eg. free listing or featured listing, and make payment by PayPal. When they have posted the listing it will be keeped draft until we have review the restaurant. We will be able to add the payment button by a short code or code. The following fields in the form: - Restaurant name - Description - Address Line 1 - Address Line 2 - Restaurant Timing Info (opening hours) - Phone - Email - Website - Twitter - Facebook - Keywords (Tags) - Add Image - Select Payment Plan Some of the fields (custom post types) need to be added. The form should be able to be added by a short code to any page. We need also be able to translate the form by PO file into Swedish. Final deliverables are to be images, HTML, JS and CSS files containing the plugin, including source files and graphic design elements. All rights will be transferred to us.
