select search form integration

I have a javascript form that functions much like the select boxes here... [login to view URL] It has a php/mysql backend. Right now the form doesn't submit anywhere. The form needs to submit to a cs-cart (smarty-template) backend search. Either by passing the varaibles from hidden fields or changing the existing select boxes. The form needs to be able to used as an include file throughout the site. This project is time sensitive, would need you to start and finish away.
