Customer input box in Magento Product

Hi All, A bit of background: we are building a new flower line at our shop. We want the customer to be able to insert the address of the person she/he want to send the flowers to at the product page, not the checkout. And also Customer should be able to insert a personalized msg (character limited). Requirements: 1) create a box for customer to insert name, address of recipient of flowers. Customer should have option (send to my address). 2) create a box for customer to add a personalized msg, with character limit. 3) The above should be done from Product page NOT at checkout. 4) This should work for all type of products, including simple, configurable and grouped. 5) the above boxes on (1,2) should be viewable from backened, preferably a PDF doc is generated or on invoice. 6) minimize the clicks for the customers. 7) this is based on the magento community edition.
