"find nearest" SQL script assistance (PHP/MSSQL)

We have a database of several hundred providers which has a location map which works fine however we have a need to tie it into a MSSQL database of providers. Each provider address has its own Longitude and Latitude reference and we are asking for a local match to be made available to search 10/25 and 50 km from a postal code or town which is user submitted.
