85765 Form with date validation

I need a form created that will mostly have standard for fields. The only advanced feature is that the form will ask the user to complete job or work history for a minimun of a two year period. The form must calculate based on the date ranges provided by user and alert the user if they have not provided at least two years worth of information. Example- The following would be ok because it contains at least two years of history. Work History Andvanced Technologies Inc. 1/1/2003 - 3/15/2004 Burger Shack 5/1/2004 - 8/1/2005 After submission the user should see a sucess page with their form input. The form the success page and input should be emailed to the user based on the user's email address provided in the form. The form results also must be emailed to a specified admin email address. Form result must be stored in MySQL database. Workflow: 1 User completes form 2 Form is validated 3 User gets success or failure page (failure specifies errors) 4 User gets success/thank you page (Link to Return to website) 5 Results emailed to User and Admin Email address 6 Results stored in MySQL database
