Form to MYSQL to excel

Hello everyone I’ll need a simple contact form which saves all fields to mysql database. The database should have a web-based interface for admin to view the data (password protected). The most important function for admin is to download mysql-data to excel file (so that each form-field is in a separate excel-cell). The admin should have a possibility to select timeframe what to download (i.e. 14.03.2007 to 17.06.2007). This can be a calendar interface or form-field. The admin can be in English, but the actual form in finnish (translation is provided). The server has Geotrust Europe SSL-sertificate and this form should transfer all data encrypted. The ready form should be installed straight to server and meet the appearance of given css-stylesheet. The form has following fields: - Name - Address - zip-code Etc. All together 10-15 fields. The project should be completed in two days. Please do not bid if there is a possibility that you are not able to execute this.
