Membership Database with multiple membership levels

I need the following database: Form to fill in up to 15 fields with the ability to restrict access as such: Chapter - enter data into fields, recall submitted forms and edit fields as new members join and leave. (only see the data that they have entered for their chapters members. State - be able to enter fields for state officers and see all chapters data in their respective state. National - all the same permissions below and the ability to see and edit entire membership database. This needs to be able to be added to our hosting and domain for our control.
