Phpmyadmin : Web-based Form with Login and Upload/Download Feature Via Excel

I need to create an online form via PHPMYADMIN that has the following features: **** I will be the admin and have control over all users. There will be multiple users.*** *** Users will be accessing the form via web browser.** 1) User can login to the page/account 2) The data in the form (image path, items, descriptions) will be populated via excel file. 3) The images will be hosted on a separate server, but will be called from the form. 4) There will be a number of selections that the user will make before moving to the next item. 5) The user can select multiple options for the same field (i.e multiple selections for "Style") 6) Each "main" category has a number of subcategories. The subcategory table will display only relevant subcategories for the main category selected. 7) Once the work is done, we can download the excel file with the completed fields. For example, -We will upload an excel with SKU, Image URL, description. -The Excel will also have 8 other columns that are blank and will be populated based on the webform selections. -The user will login and select the appropriate checkboxes for each item and move to the next item. - Each item will have 8 fields (i.e category, style, finish, etc.) and each field will have specified checkboxes. -Once done, we can download the Excel that will contain all the selected fields for each product. ***_This program must be web-based, so the user can log in and complete the fields from anywhere.
