Wordpress Form Plugin

I need a wordpress plugin that will create forms using the normal types fields found in most common form plugins but I am also requiring the ability to have the form send the user to a certain webpage after the form is submitted by the user based on the answers that are given. For example if you had the following radio button field that has the options of excellent and poor. If the individual submitting the form selected "excellent" then they would be redirected to a certain webpage or if they selected "poor" they would be directed to a different webpage after the submit the form. I also need a way to store, manage and create reports (that can be emailed to specified individuals)from the data that was collected in the form
