Creat 3 Forms in Joomla

Need to have 3 forms created that can be inserted into my joomla page. Must be able to attach a file. When submitted is clicked, the information and any attached files must then be sent immediately to an email address with the information sent as an attachment as well. Will provide sample on how the form is to look like. Do not want to have to install additional modules in Joomla. Please submit on how you would do this stating the technology proposed.
