Develop website and MySQL database

Need someone to design a simple web form and a MySQL database backend. The form is used to collect information on people with certain skills and understand certain technologies. Forms and tables should be able for me to easily adjust stylesheet. Please let me know if you can do this job and how long it will take you. Please be sure to read all information and ask questions. OPTIONAL: forms should use Ajax for submitting and loading data. There are 4 main pages: - Form for entering information. Form should allow user to upload a word document or pdf file, store the file in a folder (prepend the timestamp to the file name to ensure uniqueness), and save the file name in the table. - View for listing summary of submitted information (first name, last name, email, city, current location) Should be able to sort on any column.
