PHP survey

PHP questionnaire Need PHP script that will allow me to create dynamic surveys/questionnaires. There are two types of surveys: normal and quiz. The normal survey allows the user to answer questions with any reply. The quiz survey adds logic to the questions and checks the response against accepted answers. See the attached PDF for the design. No fancy graphics are required. Standard jQuery would work. There are two types of questions, multiple choice and free form, each with two variations. - Multiple Choice : Single Answer (radio buttons) - Multiple Choice : Multiple Answer (checkboxes) - Free Form : Single Textbox - Free Form : Comment / Textarea The site should use javascript/ajax/jQuery when possible to dynamically display and update information. The information should be stored in a MySQL database. You can come up with you own design for how to store surveys, questions, and results. I just need to be able to query the database to get the submitted results at the end. An example database design could look like the following: Survey Table - surveyId (int) - surveyType (varchar) - dateCreated (timestamp) - dateLastUpdated (timestamp) Questions Table - surveyId (int) - questionId (int) - questionType (varchar) - question (text) - answers (text) Results Table - surveyId (int) - dateSubmitted (timestamp) - questionId (int) - answer (text) Need this ready in 3 days - by Saturday, November 24 at 2pm (GMT-5). I currently have a PHP based site that does something similar and if needed, could be adapted. The site allows me to create trivia questions that are used by a different system. However, the question types are all the same - multiple choice w/ one answer, each has up to 4 options. The system does not store results. Possible answers for each question are stored in the database comma delimited with an indicator in front of the correct answer. For example: answer1,answer2,>answer3,answer4 Answer3 would be the correct answer for the question. MUST: - check that all SQL queries are protected from SQL Injection hacking - document all php functions
