Form Example: Patient Info: Name, Doctor's Name, Date, Email, Phone, Age, Race, Weight, Height, etc. The Form will have about 20 questions, Example: Q1: How would you rate your improvement on your test? 1 - 10 (1 is worst, 5 is same, 10 is Excellent) Q2: Do you wish to continue use the medicine? Yes, No Report System: Doctors and Admin can see the report about the result for each question. Q1: 8.5 Q2: Yes: 93%, No: 7% Admin can view report for combine result of all doctor's patient data.
