Quick Turnaround - PSD to Html/CSS - Find/Install Setup Wordpress Plugin for 5 Forms

2-3 Days to complete this project only... if any longer please do not respond. Ready to START NOW! Take an already existing website built with a static design with html css and place the static header and footer into a wordpress blank theme (using [login to view URL] ) We do NOT need to recreate any of the existing site menu or header or footer elements using wordpress. This is a quick job and we don't need that. The other tasks involve the following: PSD to HTML for the inside content on the following pages (Do not include time to code the header and footer as the code is already done) Find a Plugin for wordpress that manages creating forms. We have 5 forms to create with this plugin. The plugin needs to be able to print the submission after they are entered. Find a plugin for wordpress to allow css popup for creating an account with wordpress user system. We need the users to register and log into wordpress as lowest level user and fill out the form. We need to theme this popup according to mockup below. PSD to HTML for the landing page after signed in to choose a form to fill out. Thank you for your time.
