JQuery and Php Form

We are holding a Conference. We want a page where someone can register themselves and/or as many people they like. For Page #1 (see image), I would like a dropdown menu with the numbers 1 to 15 and a Go button. Once the user clicks on the 'Go' button, X number of forms will appear on the page for the user to fill out. The first one will be visible and the rest collapsed. At the bottom there will be a continue button. This will Link to the next page. On the next page will be a summary of the order (Again see image). # of attendees the price, taxes, discount and Total. I need price, taxes, and discount to all be variables that I will change later. For Now lets say Price per Attendee is $595. Taxes 5% and discount $50. There will be validation on attendees. The user will have two option to Pay by: Credit Card or Invoice. Both will link to a third page. For Invoice, it is a new page with the the same form as the first page. It will be pre-populate with the first attendee. (The assumption is the first attendee is the person to invoice). The user clicks a button submit invoice, and email will be send to them describing they will receive an invoice shortly and another email sent to the administrator with the person to invoice, and all the attendees. they will be redirect to a 4th and final page which will thank them. On the server site I will have an array of the attendees when I can add them to the database. For Credit Card, they will push to a page, where I will have an array where I can enter information into the database, then redirected to [login to view URL]://www.recruiternetworkinggroup.com/Conference2010/approved.php&trnAmount=XXXXXX This page will be pre populate with the first attendee. (The assumption is the first attendee is the person to invoice). As of right now variables get pay to this page by $_GET Lastly for the Credit Card, they will be redirect to [login to view URL] (or whatever page the $_GET['approvedPage'] is) Attached is the image to describe why I want better.
