250892 Help with submit form

Hello, I need some help with a form sumbit item so if you submit a zipcode it would place the zip code in the url string and perform the search when the submit button is clicked. I would like a simple form created Stating enter zip code then submit so the search string is set in place and would place the entered zip code in there.
