I need a 3 page application form completed which should be falrly simple. The user will be applying for financing and the form will collect the users information and email the information back to me. The first page of the application form should collect the users basic information (also allow for a co-signer to fill out info as well), the second page should give the user the option to choose the amount of financing they need, the last page will be a "Thank you for your submission" page advising the user that we recieved their information. The form should flag users when information is missing. Please test the form to gauge if you can complete the form. Lastly, please provide samples of your relevant work with your bid?

Hey Bobby, I can do this form for you no problem. The example you showed is done in Javascript and that would be the best way to get the job done. I would need all the data fields that you would want on each page of the form to get started. The returned data could be emailed or added to a MySQL database for retrieval later. I can make this a stand alone page, or something that you can just include in another php page.
