Add page to Customer Area of Magento

We have a Magento site online but we need an additional section added to the "Customer" area. We would like a page added for members which will allow them to fill in a form made up of around 10 fields, select an Image from there computer and then press submit to send all of this to the site owner. The file attachment is very important for this. The site owner will receive an email with all of the Form fields information, the attachment and the Members Name and e-mail address. Our usual Magento programmer has been taken ill so we are desperate to get somebody to complete this quick job ASAP, it will lead to more work for the right candidate.
