Tomato Cart Customization - Adding a form and button

I need a button < Make me an Offer > on cart page as well as n product listing and detail page (when laptop category is Faulty Laptops). Button is attached. Upon clicking this button a form should pop up containing below fields Name : Address : Email : Phone number : Selected Product(s): Price offer: Valid Paypal Address : Comments : All fields should be required to be filled. All the form data should be emailed to my id. There should only be one category added to cart at one time. If a user added a faulty laptop to cart he can't add a Refurbished laptop to cart. What i mean is products of only one category can be added to cart at a time. User can offer me price of one faulty laptop or he can add to cart more than one faulty laptops and make me a commutative price offer there in the cart. [login to view URL] For the product listing page if the category is "Faulty Laptop" there should be "Add to cart" and "Make me an Offer" button instead of "Buy Now " button. [login to view URL] For Single Product page there should be "Make ma an Offer" button along with "Add to Cart" button [login to view URL] In Cart, if the products are from Faulty Laptops category there should be "Make me an Offer" instead of "Checkout" button. Please let me know if you have any query. I need this done asap so no time wasters please.
